Spondylolisthesis and Nerve Pain: Neck and Lower Back

A vertebra that's slipped out of alignment can narrow the space nerves travel through in both the neck and lower back, and in this condition, functional rehabilitation isn't a bonus add-on, it's the core of recovery.

What Spondylolisthesis Actually Is

Spondylolisthesis describes a vertebra that has slipped forward, narrowing the space for spinal nerve compression., or occasionally backward, known as retrolisthesis, relative to the vertebra directly below it. It's graded from I to IV based on how far the vertebra has shifted, and while it can occur at any level of the spine, it shows up most commonly in the lower back, particularly at the L4-L5 and L5-S1 levels, with cervical spondylolisthesis being considerably less common but still clinically significant when it does occur.

Causes vary. In younger, active people, the nerve symptoms it produces are often mistaken for straightforward sciatica, another reason why the label 'sciatica' matters less than finding the cause., it's often related to a small stress fracture in a bony bridge at the back of the vertebra, a condition called spondylolysis, which can progress to spondylolisthesis over time. In older adults, it's more commonly degenerative, the result of long-term wear on the discs and facet joints that gradually allows one vertebra to shift relative to the next.

How It Irritates Nerves in the Lower Back

When a lumbar vertebra slips forward, it can narrow the space available for the nerve roots exiting the spine at that level, or reduce the overall diameter of the central canal that the nerve roots travel through. This narrowing is what tends to produce the classic symptom pattern, pain, tingling, numbness, or weakness radiating into the buttock and leg, often following a sciatic distribution.

A distinguishing feature of lumbar spondylolisthesis-related nerve irritation is how it responds to position: symptoms are frequently worse with standing and walking, and eased by sitting or bending forward, because flexion tends to open up the space the nerve roots are travelling through, while extension narrows it further.

How It Irritates Nerves in the Neck

Cervical spondylolisthesis is discussed far less often than its lumbar counterpart, but the underlying mechanism is similar, a vertebra shifting out of alignment narrows the space available for the cervical nerve roots, or in more significant cases, the spinal cord itself. Symptoms can include neck pain alongside radiating pain, tingling, or numbness into the shoulder, arm, or hand, depending on which level is affected.

Because the cervical spine houses the spinal cord rather than just nerve roots, more significant cervical instability carries a genuine risk of cord-level involvement, sometimes producing symptoms like hand clumsiness, balance changes, or an altered gait. These are signs that warrant prompt medical assessment rather than a purely conservative approach, which is part of why a thorough neurological screen is such an important part of assessing any suspected cervical spondylolisthesis.

Why Functional Rehabilitation Matters So Much Here

Because spondylolisthesis is a structural finding, the vertebra has already shifted, treatment isn't about physically moving the bone back into place. It's about building enough stability around the affected segment that it's no longer moving excessively under load, which is often what's driving the ongoing nerve irritation in the first place. This is why functional rehabilitation tends to be the central part of a spondylolisthesis plan rather than an afterthought.

Deep core and multifidus stabilisation, developmental-kinesiology-based approaches like DNS, and controlled, staged loading all aim to improve how well the spine controls movement at and around the unstable segment. For lumbar cases in particular, moderating excessive spinal extension under load, a movement that tends to narrow the space for the nerve roots, is often an early priority, while still building general strength and capacity over time.

What a Considered Treatment Plan Looks Like

A typical approach combines hands-on care aimed at reducing symptoms and mechanical irritation in the short term, manual therapy, soft tissue work, and spinal decompression where appropriate, with a longer-term functional rehabilitation program focused on stability, motor control, and graduated strength. The balance between the two shifts over time, with rehabilitation becoming the dominant focus as acute symptoms settle.

Most cases of spondylolisthesis are managed conservatively, though outcomes depend on the grade of slippage, the level involved, and how the individual responds to a structured program. Progressive neurological symptoms, significant instability, or a lack of meaningful progress with conservative care are the main triggers for a specialist or surgical opinion, which is why ongoing reassessment throughout a rehabilitation program matters just as much as the initial diagnosis.
