Same Symptoms, Different Origins

Patients often arrive assuming they know which one they have, based on something they read online, when the actual picture, confirmed through proper examination, turns out to be different.
The confusion is understandable. Both can produce similar sensations, vitamin deficiencies that cause nerve symptoms also enter this differential. in similar locations. But sciatica describes irritation of a specific nerve root as it exits the spine, while peripheral neuropathy describes more widespread nerve dysfunction, often affecting multiple nerves and frequently starting at the furthest points from the spine, the feet and hands, rather than radiating from the lower back.
How Sciatica Typically Presents
What sciatica actually is, it usually follows a specific pathway, pain, tingling, or numbness travelling from the lower back or buttock down the back of one leg, often following the course of the sciatic nerve fairly precisely. It's frequently aggravated by specific positions or movements, such as prolonged sitting, bending forward, or coughing, and it's typically one-sided.
Because sciatica stems from a mechanical source, a disc bulge, spinal stenosis, or piriformis-related compression, for example, symptoms often have a clear relationship to posture, movement, and load, and that relationship is one of the clues we use during assessment to help confirm the diagnosis.
How Peripheral Neuropathy Typically Presents
Peripheral neuropathy tends to look different. Rather than following one nerve's pathway down a single leg, it often presents symmetrically, both feet, or both hands, affected in a similar 'stocking and glove' distribution. It's less commonly tied to a specific movement or position, and it can have causes unrelated to spinal mechanics altogether, including metabolic conditions, certain medications, vitamin deficiencies, or other systemic factors.
Because some causes of peripheral neuropathy fall outside what a musculoskeletal assessment alone can identify, this is one of the areas where working alongside your GP matters, bloodwork or further investigation may be needed to identify or rule out an underlying systemic cause, in addition to any mechanical component being addressed through treatment.
Why the Distinction Changes Treatment
Mistaking one for the other doesn't just risk a wasted appointment, it risks a treatment plan that's aimed at the wrong mechanism entirely. A mechanically driven sciatica case tends to respond to identifying and reducing the source of nerve root compression, combined with movement-based rehabilitation. A neuropathy with a systemic driver needs that driver addressed as part of the picture, alongside any supportive musculoskeletal care.
This is exactly why a proper neurological and orthopedic examination, checking reflexes, strength, sensation patterns, and how symptoms behave with specific movements and positions, matters so much before treatment begins. The distribution and behaviour of your symptoms tells us far more than the general description of 'nerve pain down my leg' ever could.
