Saphenous Nerve Entrapment: Medial Knee and Leg Pain

The saphenous nerve can be compressed at several different points along its long path down the leg, and Hunter's canal is only one of them.
A Long Path, Several Possible Pinch Points
The saphenous nerve is a sensory branch of the femoral nerve with an unusually long journey, making it one of the more overlooked forms of nerve entrapment in the leg., travelling from the groin, through the thigh, past the inner knee, and down to the inner ankle and foot. Because of that length, it can become compressed at more than one point along its course, and saphenous nerve entrapment is really an umbrella term covering irritation at any of these sites, rather than one single condition.
Hunter's Canal: The Most Common Site
The adductor canal in the mid-thigh, also known as Hunter's canal,'s canal, is the most frequently affected location and is significant enough to be discussed as its own condition. But the saphenous nerve can also become irritated further down, where it emerges near the inner knee between two tendons, or lower still, near the inner ankle, each site producing a slightly different pattern of symptoms depending on where along the nerve's path the compression sits.
How It Presents
Regardless of the specific site, saphenous nerve entrapment typically produces burning, aching, or numbness. Meralgia paraesthetica affects the outer thigh via a different nerve and is a useful comparison., aching, or numbness along the inner thigh, knee, shin, or ankle, without any associated muscle weakness, since this is a purely sensory nerve. The exact location of symptoms is one of the more useful clues in narrowing down which part of the nerve's pathway is actually involved.
What Contributes to It
Direct trauma or surgery near the inner knee, tight adductor muscles compressing the thigh portion of the nerve, and repetitive activities that load the inner leg, like cycling or activities involving a lot of hip and knee flexion, are among the more common contributing factors across the different entrapment sites.
Locating the Source
Because the nerve can be compressed at multiple points, careful clinical examination, palpating along the length of the nerve's pathway and using specific provocation tests at each potential site, is what allows an accurate diagnosis. Once the specific location is identified, treatment can be targeted directly at that site rather than applied generally along the whole leg, which tends to produce considerably better results.
